PAYMENT POLICY

No Insurance

A 30% discount will be given to patients for elective procedures if payment is received in full at the time services are rendered. For ER, OBS or IP admissions, patient will be given 30% discount if paid in full within 20 days, to include holidays and weekends, of 1st time statement.

Insurance

A 20% discount will be given to patients for any services that are rendered and are not paid for in full by the insurance plan. This payment must be paid in full within 30 days, to include holidays and weekends, of the 1st statement after insurance has been paid.

ALTERNATE PAYMENT OPTIONS - MINIMUM PAYMENT SCALE

Debt <=$500 $50 monthly

Debt >$500 to $1,000 $75 monthly

Debt >$1,000 $100 monthly

All accounts shall be paid within 12 months of the date of service. Minimum payments on balances over $1,000 are calculated by dividing the balance by 12 to determine the minimum monthly payment. Accounts are considered delinquent if not paid in full or if satisfactory payment arrangements are not made with 60 days of the date of service.

Payments below minimum payment scale will be considered delinquent and will be sent to a collection agency and may damage your credit.
